Each item's first picture is its measured drawing and the second a room plan with the door width and 90 cm for the carer. The specs say which level of patient it's for, how much space it takes, who sets it up, the power it draws and the weight it bears; each model is an option with its own price.

Where do I change an item's care level and size?

Both are product attributes, changed on that product's page in the panel. Which attributes show on cards and which become filters you pick in the shop settings.

Does it have an English site?

Yes. Every page and product takes an English translation and the visitor switches language from the header.

What else do I need to sell online?

A payment gateway in your own name and a domain. Hosting, upkeep and updates are on Sima.
